Caregiver Support
Our caregiver support program is designed to equip caregivers with the tools and strategies needed to support their child’s progress. Through virtual and in-person meetings, parents learn to implement evidence-based techniques consistently in their daily routines, fostering a supportive environment that promotes skill development and behavior management. This training helps parents understand the principles of evidence based treatment such as positive reinforcement, visual supports, and functions of behavior, enabling them to address challenging behaviors and encourage positive ones. Our caregiver support sessions ensure that the benefits of the program extend beyond formal sessions, leading to more significant and lasting improvements in the child’s overall quality of life.

Horizons serves the families of children, adolescents, and teens from ages 2-18. While our strategies can be applied to both neurotypical and neurodiverse individuals, our team of Behavior Analysts has years of experience working with children and caregivers who need individualized care, including those with diagnoses such as Autism Spectrum Disorder, Attention Deficit Hyperactivity Disorder (ADHD), emotional and behavioral disorders, and Oppositional Defiant Disorder.

Our services are meant to be flexible, as we recognize each families’ needs are different or change over time.
Primary areas of support include:
Increasing independence in daily routines
handwashing, toilet training, bedtime routines
Increasing success in community outings
doctor visits, haircuts, trips to grocery store
Facilitating communication methods
Understanding or helping others to understand the individuals’ diagnosis
Managing and decreasing maladaptive behaviors
hitting, biting, self injurious behaviors such as head banging or skin picking

Horizons offers services through Wisconsin’s Children’s Long-Term Support (CLTS) Waiver Program which provides services to children with developmental, cognitive, and physical disabilities. More information on the CLTS program can be found here: https://www.dhs.wisconsin.gov/clts/eligibility.htm
Horizons also accepts private pay.

After ensuring your child or teen is approved for CLTS support, services can be sought out by caregivers or referred through county caseworkers. Caregivers can use the “Get Started” tab below to submit their information. Caseworkers can fill out a referral form and submit it to info@horizonsbehavioralhealth.net.
Program Details
Our Caregiver Support Program is designed with weekly sessions for 12 weeks, delivered virtually via video call. First, a Board Certified Behavior Analyst (BCBA) will conduct an initial intake meeting, working with caregivers to determine their current priority areas of support. After identifying these areas, sessions will be scheduled to begin working towards the goals stated by caregivers. Throughout the 12 weeks, if it is determined that the caregiver and client would benefit from in-person support, sessions in the home or community will be scheduled for at least once monthly in the counties of Portage, Waupaca, Outagamie, and Calumet counties. After the 12 week program, the need for support will be reassessed.

We are considering and hoping to launch groups such as social skills, daily living skills or recreational/leisure groups designed to help individuals with autism and developmental disabilities enhance their social interactions, to foster relationships within the communities that the individual lives in and to HAVE FUN!
